Cost of Chronic Care Management
As with almost all worthwhile services, a cost is associated with Chronic Care Management. The cost each patient has to pay varies based on their insurance coverage. Some patients don’t pay anything if they have the proper secondary insurance. The way Chronic Care Management gets billed is dependent on how much care time is given to the patient each month. The time is split into 20-minute segments, where any care time in a month being under 20 minutes will not get billed. CPT codes are used for billing, with each code representing how much time was spent on that patient in a given month.
